This WEB RESERVATION SERVICE is available not only for first-time visitors, but also for return visitors who have been coming to the clinic for more than two months since their last visit.

A. There is a dedicated parallel parking lot next to the building (yellow box). Please fill out the form in front of the janitor’s office on the first floor.
If the parking lot is full, please use a nearby parking lot and present your parking ticket. A flat rate will be charged for the use of the parking lot.
